The Advanced Diploma in Nursing (Community Health) course will offer the following modules:
(1) Evidence-Based Practice (45 hours)
(2) Population Health for Older Adults (60 hours)
(3) Health Needs of Families and Vulnerable Populations and Community Resources (60 hours)

What You'll Learn
At the completion of the PDC, learners will be able to:
(1) Discuss the care for clients with chronic health conditions in intermediate and long-term care settings using principles and concepts of care management;
(2) Apply evidence in community health nursing in the development of community health nursing practices using principles of evidence-based practice and appropriate appraisal framework;
(3) Develop healthy ageing programs in the community with reference to the biopsychosocial model; and
(4) Identify appropriate community resources to meet the health needs of families and vulnerable populations using the concept of interdisciplinary care.
Minimum Entry Requirement
- Diploma in Nursing from a local Polytechnic or an equivalent qualification;
- Singapore Nursing Board registration;
- 2 years post registration experience with a minimum of 1 year in the discipline of study.
