Through practical insights and real-world case studies, learners will also grasp the significance of health and social care integration and discover how community-based initiatives can enhance the overall well-being of patients/ clients.
Social Prescribing & Asset-Based Community Development – Part of Post-Diploma Certificate in Holistic Approach in Health and Social Well-Being (SDIS) is a 45hrs module.
Applicants must have a local Polytechnic Diploma or University Degree in any discipline with at least 1-year relevant working experience.
