The objectives of this Part-Time Diploma are to enable students to:
- Coordinate and navigate care of clients in order to ensure seamless transition of care;
- Coordinate all aspects of discharge planning;
- Act as a liaison between patients, family members and the multidisciplinary team (MDT), institutions, external agencies, and the community;
- Work with community partners to support patients in achieving individual health improvement;
- Connect individuals with appropriate health and social services to make best use of community resources to tackle health issues.
- Assist individuals to change their health behavior to improve their own health and wellbeing;
- Promote health in the community through the application of behaviour change strategies; and
- Conduct and evaluate appropriate screenings for the community to establish health and fitness assessments.
What You'll Learn
In Singapore, the expanding longevity among seniors and fertility decline are contributing to the greying of its population. By 2030, one in four Singaporeans living amongst us will be aged 65 years and above. This silver tsunami has created new and increasing demands on Singapore's healthcare system because it is associated with a higher prevalence of chronic health conditions among seniors. Seniors, specifically those with chronic conditions, often lack sufficient knowledge about their condition, health promoting behaviours like regular exercising and healthy nutrition, thus resulting in poor self-care skills. Hence, this course will help you understand seniors better, promote healthy behaviours including exercise and nutrition and prepare you to work with a multidisciplinary team of healthcare staff to keep seniors living in the community healthy.
Minimum Entry Requirement
O Levels
- 3 relevant passes; and
- At least 1 year relevant work experience
Higher Nitec
- GPA ≥ 2.0; or
- GPA ≥ 1.5 and at least 1 year relevant work experience
Nitec
- GPA ≥ 3.5; or
- GPA ≥ 3.0 and at least 1 year relevant work
Higher Nitec in Technology / Services
- GPA ≥ 2.0 and at least 1 year relevant work experience
Nitec in Technology / Services
- GPA ≥ 3.5 and at least 1 year relevant work experience
WSQ
- Relevant WSQ Advanced Certificate (or higher); and
- At least 1 year relevant work experience; and
- Level 6 WSQ Workplace Literacy SOA and Workplace Numeracy SOA
Applicants who do not meet the entry requirements may be considered for admission to the course based on evidence of at least 2 years of relevant working experience. Suitable applicants who are shortlisted will have to go through an interview and/or entrance test. The Polytechnic reserves the right to shortlist and admit applicants.
